Expired Medications Found in Medication Carts
D
F0761 F761: Ensure drugs and biologicals used in the facility are labeled in accordance with currently accepted professional principles; and all drugs and biologicals must be stored in locked compartments, separately locked, compartments for controlled drugs.
Short Summary

Expired medications were found in two medication carts and in the PRN drawers for several residents, including saline nasal gel, loperamide, lactulose, and multiple bisacodyl products. Staff said cart checks were done inconsistently, the DON expected weekly checks, and facility policy required expired drugs to be removed and medication expiration dates to be reviewed before administration.

Failure to Post Daily Nurse Staffing Information
C
F0732 F732: Post nurse staffing information every day.
Short Summary

Failure to Post Daily Nurse Staffing Information: The facility failed to keep the daily nurse staffing sheet updated and posted as required. The staffing information in the common area remained dated and unchanged for multiple survey observations, despite the facility policy requiring daily posting. RN and DON interviews confirmed the nightshift was responsible for updating the sheet, but agency staff had been working and the information had not been updated. The Administrator stated the staffing sheet was supposed to be posted daily so staff and families would know who was in the building.
