The DM and DON stated they were responsible for checking the refrigerator, but it had not been checked daily, and the Administrator stated unlabeled or undated food items were to be discarded.

The facility failed to store, label, and date food in accordance with professional standards for food service safety. Review of the facility policy titled, Storage Procedures, revised 01/2025, showed that refrigeration equipment was to be routinely cleaned and defrosted, foods were to be covered and dated, leftovers were to be refrigerated immediately and used within five days with a clearly marked used-by date, and individual frozen supplements were to be dated if stored in refrigerators on designated nurses’ station kitchen refrigerators. During observation of the Nutritional Room refrigerator on the 100 hall with the RN, surveyors found a peanut butter and jelly sandwich that was not labeled or dated and had hard bread, a loosely wrapped partially eaten restaurant sandwich that was not labeled or dated, a bagged meal from a fast-food restaurant with a receipt dated 09/16/2025, a plastic container of leftover food that was not labeled or dated, three bottles of coconut water with an expiration of July 2025, and eight Magic Cups that were melted and not labeled or dated. The DM stated she and the DON were responsible for the nutrition room and that she tried to check the refrigerator weekly, but not as much as she should. The DM, DON, and Administrator each stated items in the refrigerator should have been labeled and dated, and the Administrator stated the refrigerator was expected to be checked daily and any food items not labeled or dated were to be discarded.
