The facility failed to post daily nurse staffing information as required for 3 of the 3 State Survey Agency survey dates. The facility policy titled, General Nurse Staffing, initiated 08/29/2022, stated staffing numbers were to be posted and visible to staff and visitors daily. However, observation on 09/17/2025 at 4:45 PM showed the daily nurse staffing information sheet in the common area corkboard was dated 09/13/2025 and had not been updated. A second observation on 09/18/2025 at 12:50 PM showed the same staffing sheet still dated 09/13/2025 with the same information, confirming it remained unchanged. During interview, RN 2 stated the common area corkboard was the only known place where daily staffing would be posted and said the nightshift nurse was supposed to update the staffing sheets, although contract agency staff had worked the past couple of nights. The DON stated the daily staffing sheets were posted in the commons area by the nurse's desk and that midnight nurses were responsible for keeping them updated each day, but agency nurses had been working the past few nights and that was why the information had not been updated since 09/13/2025. The DON later stated her expectation was for the daily staffing sheets to be posted daily with correct information, and the Administrator stated the sheets were to be done daily and posted so staff and families would know who was in the building, adding that a nightshift nurse had been responsible but had dropped the ball and the responsibility fell through the cracks.

During observations on 6/1/2026, staffing information dated 6/1/2026 was posted at the main entrance, but the census listed as 103 did not match the Resident Listing Report census of 102. Later that same day, staffing information dated 6/2/2026 was observed posted, and on 6/2/2026 staffing information dated 6/3/2026 was posted at the main entrance. On 6/3/2026, staffing information dated 6/4/2026 was posted at the 100 Hall nursing station near the off-hours facility entrance. During interview, the DON confirmed the staffing information posted was dated 6/4/2026 and stated the scheduler would prepare the staffing information sheet before leaving and corrections would be made the next day. The DON also stated the staffing sheet should not be posted in the front and should be placed behind the current day until the new shift starts. Administration stated the staff coordinator should not post the staffing information the day before and should post the current staffing information for the day. The Staffing Coordinator stated she had been putting the staffing information out before leaving at the end of the day for the following day and did not notice the census was incorrect.

The facility policy, Nurse Staffing Posted Information dated 02/01/2026, stated that nurse staffing information was to be made readily available in a readable format to residents, staff, and visitors at any given time.

Surveyors observed the posting on multiple occasions and found it dated 5/26/26 on 5/27/26 at 2:15 p.m. and again on 5/28/26 at 6:58 a.m., dated 5/26/25 on 5/27/26 at 4:10 p.m., and dated 5/28/26 on 5/29/2026 at 8:13 a.m. The deficiency affected all 24 residents residing in the facility. During interview, NA-A verified the posting was dated 5/26/26 and stated it was important for families and visitors to see how many staff were working. SC-B stated she changed the nurse staffing hours on Thursdays for the coming week but did not update the posted sheets, and instead made the changes in the computer after they occurred when she returned on Mondays. A review of the next week's schedules showed the census as 24, and one schedule dated 5/30/26 had zeros for all staff listed with a census of 24. The DON verified the nurse staff posting should be posted daily so anyone could see how many residents were in the facility and how many staff were working, and stated it was important so families could have confidence there was enough staff to take care of their family members.
