Summary

Drugs and biologicals in the facility were not maintained in accordance with accepted professional principles because expired medications were found in medication carts and in resident-specific PRN drawers. On the Short-Hall medication cart, surveyors observed AYR Saline Nasal Gel for one resident with an expiration date of 03/18/2025, Loperamide capsules for another resident with an expiration date of 09/11/2025, and Lactulose liquid and Bisacodyl EC tablets for a third resident with expiration dates of 08/07/2025 and 04/14/2025. On the Long-Hall medication cart, surveyors observed Bisacodyl EC tablets for three residents with expiration dates of 04/22/2025, 04/30/2025, and 05/19/2025. The report also identified expired medications for six sampled residents total: R5, R8, R17, R18, R30, and R39. Facility staff acknowledged responsibility for checking medication carts for expired medications, but the KMA stated she tried to check them monthly, while the DON stated her expectation was that whoever used the cart would check medications at least weekly. The contract pharmacy AD stated consultant pharmacists came monthly for medication and chart reviews and sometimes checked the carts, but not very often. The Administrator stated staff were expected to follow facility policy and remove expired medications from the medication cart. Facility policies reviewed stated expired, discontinued, or deteriorated drugs were not to be used and should be returned to the pharmacy or destroyed, and that medication expiration dates should be reviewed before administration.
