The MDS coordinator said the resident was full code on admission and later changed to DNR, but the change was missed, and the DON acknowledged the CCP was inaccurate.

Summary
Food items were not properly stored and labeled in the kitchen. During an observation on 4/14/2026, bread was found in the active food supply on trays near the back of the kitchen, including two packages dated 03/13/2026. When asked whether the date was an expiration date, the Dietary Director stated the bread was pulled from the freezer and served each day, but she could not say when it had been pulled and could not locate a label showing a used-by date. The two packages dated 03/13/2026 were discarded during the observation. The Dietary Director also reported receiving bread from the supplier that day, but there was no clear labeling showing when the bread had been received, when it had been removed from the freezer, or when it should be used by. The facility also failed to maintain the sanitizing station for the three-compartment sink in proper working order. On 04/16/2026, the Dietary Manager tested the sanitizing solution with test strips and multiple tests showed no observable color change. After additional sanitizing solution was added, the strip showed only a minimal color change and remained blue, which corresponded to 0-170 ppm on the bottle label, while posted signage above the sink indicated the acceptable range should be 272-700 ppm and another sign on the adjacent wall listed 200-400 ppm. The DM and dietary personnel #2 stated the facility had recently changed vendors and the sanitizing station had been replaced. Dietary personnel #2 also stated there was no current log documenting sanitizing solution test results for the three-compartment sink.
Care Plan Not Updated for DNR Status Change

The facility failed to review and revise Resident #6’s comprehensive care plan to reflect a change in code status from full code to DNR. Resident #6 had diagnoses of dementia, Alzheimer’s disease, and major depressive disorder, and the quarterly MDS assessment coded the resident as having problems with long- and short-term memory and severe impairment in cognitive skills for daily decision making. The clinical record included a provider DNR order dated 01/17/2026 and a DDNR order dated 10/18/2025 signed by the resident’s POA and physician, yet the care plan still listed the resident’s code status as full code with a date initiated of 10/02/25. During the survey, the DON was notified that the care plan listed full code while the record contained a DNR order and acknowledged that the care plan was inaccurate and would be corrected. The MDS coordinator stated that Resident #6 had been full code on admission, later changed to DNR, and that the change had been missed. The issue was reviewed with the Administrator and DON, and no further information was provided to the survey team prior to exit conference.
