A resident with Parkinson’s disease, weakness, and other chronic conditions was using a power wheelchair when the chair struck a curb in the facility parking lot and tipped, causing a fall with b/l SDH, C1/C2 fractures, a scalp laceration, abrasions, and a scalp hematoma. The record did not show a safety skills assessment or education for safe power wheelchair use, and staff stated no wheelchair safety evaluation had been completed on admission or after admission.

Summary
The facility failed to ensure the resident environment remained free of accident hazards and to provide adequate supervision for a resident using a motorized wheelchair. Resident #1 had diagnoses including Parkinson’s disease, bilateral hearing loss, spinal stenosis, osteoarthritis, spondylosis, anxiety disorder, weakness, depression, adult failure to thrive, and later documented cervical fractures and traumatic subdural hemorrhage. The most recent MDS showed a BIMS score of 13 out of 15, indicating the resident was cognitively intact. Clinical record review showed that on 4/03/26 the resident was seen for altered mental status with increased confusion, with a urinalysis pending and a discharge postponed because of the change in condition. Later that day, the resident was observed outside the facility in a power wheelchair, struck a curb, and the wheelchair tipped, causing the resident to fall and hit the head. The resident sustained bilateral subdural hematomas, first and second cervical vertebral fractures, a scalp laceration, multiple abrasions, and a scalp hematoma, and was transferred to a higher level of care for surgical intervention. The record did not show evidence of a safety skills assessment for use of the motorized wheelchair or education regarding safe use while at the facility. Staff interviews indicated the resident had been admitted with the power wheelchair, had used it daily, and therapy had worked with the resident on obstacle courses, but no wheelchair safety evaluation had been completed on admission or after admission. The DON and RDCS acknowledged that evaluation for wheelchair safety had not been done, and the resident’s power wheelchair was later removed from use at the facility after the incident.
