Summary

The facility failed to review and revise Resident #6’s comprehensive care plan to reflect a change in code status from full code to DNR. Resident #6 had diagnoses of dementia, Alzheimer’s disease, and major depressive disorder, and the quarterly MDS assessment coded the resident as having problems with long- and short-term memory and severe impairment in cognitive skills for daily decision making. The clinical record included a provider DNR order dated 01/17/2026 and a DDNR order dated 10/18/2025 signed by the resident’s POA and physician, yet the care plan still listed the resident’s code status as full code with a date initiated of 10/02/25.
