The facility failed to ensure a functioning call light system was available in a resident’s room and bathroom/bathing area for one of five residents reviewed, R102. R102 was admitted with diagnoses including displaced fracture of the greater trochanter of the left femur, COPD, bipolar disorder, anxiety disorder, depression, irritable bowel syndrome, and bilateral primary osteoarthritis of the knee. Her quarterly MDS showed a BIMS score of 13, indicating she was cognitively intact, and her care plan directed staff to educate her on call light use and keep the call light within reach at all times. Review of the facility policy stated that bedside call lights should produce a light and sound over the door and at the nursing station, and defective call lights should be reported immediately to maintenance. The maintenance work order log for the prior 30 days showed no documented repairs related to call light systems in resident rooms. During observations, R102’s call light button was pressed and initially produced no visual notification or audio alert to staff at the nursing station; on a later observation, the room still had no working call light with no visual notification or audio alert available inside the room; on another observation, pressing the call light triggered the light but still no audio alert. RN1 stated the light worked but staff could not hear what the resident needed through the audio system and had to walk to the room, and the DON stated that when R102 pressed the call light, the light came on and staff were expected to go see what she needed because they were unable to hear her through the room audio system.
