The cheese on the meatball sub was stiff and translucent, the french fries were 79 degrees Fahrenheit, the meatballs were 82 degrees Fahrenheit, and the cottage cheese with strawberries and bananas were 59 degrees Fahrenheit. The facility's policy required food temperatures to be recorded prior to service, but this was not done after placing the food on the steam table. This deficiency was investigated under Complaint Number OH00161296.

Additionally, there was embedded dirt in all corners and the base of the floor. The resident expressed dissatisfaction with these conditions, indicating that they had been present for some time. Further observations by the Director of Nursing (DON) confirmed similar issues throughout the facility. The walls in the hallways had multiple chips, gouges, and dirt, and the cove bases were beveled out or missing pieces in several areas. Doorways leading to the shower room and additional hallways had large areas of missing and scraped paint, with gouges and indentations on both sides of the doorway frames. A vent in the hallway was also unsecured and lifted from the floor when stepped on, revealing a hole underneath. These findings were in violation of the facility's policy to maintain a safe, functional, sanitary, and comfortable environment, as outlined in their Resident Environmental Quality policy dated 11/29/22.
