Summary

The facility failed to maintain smoke barrier walls, as evidenced by a penetration in the first floor smoke barrier wall next to a steel beam, above the smoke doors leading to the C2 Wing. This deficiency was observed on December 9, 2024, at 11:05 a.m. The issue affected two of the nine smoke compartments within the facility.
