During clinical record review, surveyors could not locate evidence that each resident’s advance directive had been reviewed with either the resident or the resident’s representative. In an end-of-day meeting on 12/29/25, the administrator, RN #1, and the social worker/administrator-in-training discussed the issue with surveyors. The facility policy titled Advance Directives, effective 09/01/24, stated that individuals would be informed of their rights regarding health-care decisions, including the right to execute an advance medical directive, and that DNR status would be discussed upon every admission. The social worker/administrator-in-training stated that advance directive information was discussed on admission, with a change in status, and upon resident or family request. During a follow-up meeting on 12/30/25, the social worker/administrator-in-training stated that no resident currently had an advance directive, and the administrator stated, “We have had them in the past, but not currently.”

During clinical record review, surveyors were unable to locate information on the residents’ comprehensive care plans that referenced their current code status. In an end-of-day meeting on 12/29/2025 with the Administrator, RN #1, and the Social Worker/Administrator in Training, the issue was reviewed, and RN #1 stated they did not think the code status had been included on the care plans. On 12/30/2025, RN #1 provided the survey team with the facility’s policy titled, Treatment Planning, which stated that the purpose of the Comprehensive Treatment Plan is to involve the individual in developing an interdisciplinary services plan document and process that is person centered. Later that morning, in another meeting with the Administrator, RN #1, the Social Worker/Administrator in Training, and the Unit Nurse Coordinator, the Unit Nurse Coordinator stated that the individuals had not been care planned for code status but were now care planned.
