Facility staff failed to store food in accordance with professional standards for food safety in the main kitchen. During an initial tour of the kitchen on 2/2/2026 at 4:22 p.m., dietary staff were asked about the expectation for opened dry goods and refrigerated items, and OS #1 stated that opened items should have an open date and used by date when the product is opened. In the walk-in refrigerator, surveyors observed an open container of shallots in liquid and a container of leftover rigatoni with no label indicating a use by date. An unopened gallon of 2% milk was also found expired past the use by date and accessible for distribution. The dry goods area also contained multiple opened items without required labeling, including 3 five-gallon buckets of rice, brown rice, and jasmine rice, an opened box of chocolate chips, and an opened half-used loaf of bread without an open or use by label. The kitchen’s bulk sugar, flour, and bread crumb bins were also observed without labels indicating open or use by dates. OS #2 stated that all of the items mentioned should have a label indicating an open and used by date. A facility policy titled Food Storage stated that when a food package is opened, the food item should be marked to indicate the open date, and leftover food should be stored in covered containers or wrapped carefully and securely and clearly labeled and dated.
