Failure to Attempt Gradual Dose Reduction for Psychotropic Medication

The Director of Nursing confirmed that while psychotropic medication reviews occurred at least quarterly and GDRs were discussed in monthly meetings, there was no record of a GDR attempt for this resident.

The facility did not establish and maintain an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment and to help prevent the development and transmission of communicable diseases and infections. During hallway meal service, staff were observed carrying uncovered dessert and fruit cups down the hallways while meal trays were being delivered to resident rooms. On 9/2/25, an uncovered dessert on a meal tray was walked down the hallway to one room, and later uncovered desserts were served to two other rooms while the meal tray cart was positioned in the hallway between rooms. On 9/3/25, the Administrator was observed opening the meal tray cart doors during hallway lunch service and leaving the doors open before walking away. On 9/4/25 during breakfast service, the meal cart was parked between rooms and multiple uncovered food items, including cut up oranges and fruit cocktail, were served to several resident rooms. The Lead stated that the facility usually leaves dessert cups or fruit cups uncovered on meal trays, and the Dietary Manager stated that the facility normally leaves dessert plates and fruit cups uncovered on meal trays and that staff should take the cart from resident room to resident room and deliver each meal.
