The CNA removed linens from the room in a red bag and took them to the soiled linen room without placing the red bag into a clean bag before transporting it down the hallway. On 3/4/26, the Laundry Staff stated that soiled linens from an isolation room would be placed in a red bag and then into a large trash bin, and that she only wore gloves when handling soiled linens. No gowns were available in the soiled linen room. The DON/IP stated that soiled laundry would be placed in a sugar bag and that laundry staff did not have to wear gowns because the sugar bag prevented them from handling the soiled linens. The Administrator stated that soiled laundry from an isolation room was placed into a sugar bag, then a red bag, then a large laundry bin, and that laundry staff would not wear gowns because the sugar bags were used. The CNA later stated she handled the soiled linens in the isolation room with soiled gloves, placed them in a sugar bag, then a red bag, and transported the bin to the soiled linen room. The RNC stated there was potential for contamination on the sugar bags. The facility also failed to conduct an annual review of its Infection Prevention and Control Program; the IPCP policy reviewed had a revision/review date of 10/22, and the DON/IP and Administrator stated they were not sure when the policies were last reviewed, with the Administrator later stating the facility had not reviewed the Infection Prevention Program and did not have a newer policy.
