The facility failed to ensure medications were not pre-poured prior to administration for 3 of 3 residents observed during medication administration. The facility policy titled, Medication Administration Policy dated 01/01/26, stated that medication shall be prepared and administered safely, accurately, and in accordance with physician orders, professional standards of nursing practice, and applicable federal and state regulation, and that pre-pouring is permitted only for a single resident. During observation on 04/01/26 at 7:45 AM, three medication cups labeled with residents' last names were found in the medication cart drawer, each containing tablets and capsules for Resident 137, Resident 132, and Resident 165. Before administering the medications, RN1 provided empty pill pouches next to each resident's medication cup, and the surveyor used those pouches to determine the contents of the cups. When asked about pre-pouring medications for multiple residents, RN1 stated, "I always prepare my medications this way. It helps me stay on time," and said she was the only one with the keys to the cart. The DON was present during the medication administration observation, and after the DON left, RN1 stated, "I think she was telling me I shouldn't have prefilled the medication cups."
