Short Summary

Expired medications, including a bottle of Tramadol, were found stored in a medication cart, and pre-poured, unlabeled topical creams were left in the cart and on top of it. An RN stated the creams were for two residents who were unavailable at the time, and an LPN confirmed the expired medication should not have been present. Facility policy requires expired medications to be removed and prohibits leaving medications pre-poured and unlabeled in carts.

A nurse failed to properly sanitize a blood glucometer and use a clean barrier during medication administration for a resident with type 2 diabetes. The nurse used an alcohol prep pad instead of the facility-approved Sani-Wipe disinfectant wipes, contrary to the facility's infection control policy. The Assistant Director of Nursing confirmed the correct procedures were not followed.
