During a kitchen observation with the Food Service Director, surveyors found multiple prepared and frozen food items in the reach-in freezer that were not properly labeled or dated, including an unidentifiable item in a blue plastic bag with received and opened dates, leftover cooked bacon labeled only with a date, an opened plastic bag of unidentifiable items with two dates, an opened package of frozen items loosely wrapped with foil and no label, unidentifiable frozen items in clear bags with and without dates, turkey burgers loosely wrapped and labeled with a date, two clear zipper bags of unidentifiable items dated 10/24/25, an unidentifiable frozen food item in a loosely tied bag without a label, frozen crinkle fries dated 1/17/26, and frozen pasta shells dated 1/7/26. The facility policy required proper dating and labeling of all food items, including ready-to-eat products, with opened items labeled with the date opened and expiration date, and leftovers and ready-to-use items allowed for 48 hours after opening. On interview, the Food Service Director stated the stored food items should have been labeled with an expiration date and that staff would need reinforcement and in-service education on proper storage and labeling of open packaged foods. Dietary staff interviewed stated that opened packaged meats are supposed to be wrapped and labeled with the date opened and used-by date, and that leftover prepared foods, if stored, should be labeled and used within 48 hours. One dietary staff member acknowledged that the meat stored in the reach-in freezer should have been wrapped and labeled with a used-by date and admitted the items were missed and that it was an error on their part. Another dietary staff member stated they were aware that proper packaging and labeling of opened food items includes a used-by date, but did not know why the listed items were found without proper labeling.
