Unsafe Maintenance of Oxygen Equipment and Personal BP Devices
Summary
The facility failed to maintain essential equipment in safe operating condition for one sampled resident and three nonsampled residents. During the initial tour, Resident 52’s oxygen concentrator was observed at bedside with grayish, dust-like particles, and Resident 52’s record showed the resident had no capacity to understand and make decisions. Later observation with RN 3 confirmed the oxygen concentrator still had grayish dust-like particles. Resident 231’s oxygen concentrator was also observed at bedside with grayish, dust-like particles, and Resident 231’s record showed a physician’s order for continuous oxygen at 2 to 3 liters per minute via nasal cannula. LVN 12 later verified that Resident 231’s oxygen concentrator had grayish dust-like particles. The facility also allowed staff to use non-facility issued blood pressure machines for resident assessments. During medication administration observation, LVN 4 used a personal automatic wrist BP machine brought from home to obtain Resident 171’s BP, and the device had the LVN’s first name written on it. LVN 4 stated the machine had not been checked by maintenance to ensure it was functioning properly. During another medication administration observation, LVN 11 used a personal automatic wrist BP machine and then a personal automatic arm BP machine to check Resident 235’s BP, and the devices were labeled with the LVN’s first name. LVN 11 also stated the machines belonged to her, were bought from a store, and had not been checked by maintenance to ensure proper function. The DON stated the facility should provide medical equipment for resident assessments and that staff should not use personal equipment brought from home.
