Summary
The facility failed to keep mechanical lift equipment and slings in safe operating condition. During the survey, an Invacare Reliant 450 mechanical lift was observed with a handwritten sign stating it was out of service, and the facility did not have maintenance documentation explaining why it was out of service. Two Invacare Reliant 600 mechanical lifts were also observed, but the facility did not provide records showing that the lifts were routinely inspected and maintained on a monthly basis as described in the manufacturer’s instructions. A witnessed fall involving a resident occurred during a Hoyer lift transfer when the sling snapped. The incident record stated that the resident fell to the floor after the sling broke during transfer from bed to chair.
