Failure to Maintain Required RN Coverage: The facility failed to ensure 8 consecutive hours of RN coverage on one day reviewed in the PBJ staffing data. The DON stated there were managers on duty but no RN that day, and later confirmed the facility had no RN coverage policy and no staffing waivers in place. The Staffing Coordinator said she tried to ensure an RN was scheduled each day, but evidence of RN coverage was not provided for the triggered date.
Failure to Maintain New Hire COVID-19 Vaccination Status Documentation: The facility failed to maintain documentation of COVID-19 vaccination status for newly hired staff. The IP stated HR had been responsible for screening new hires but did not follow up on vaccination status or maintain documentation, and the HR Director stated the facility did not collect or maintain a staff list of COVID vaccination status. A policy review showed the facility would provide education on the benefits of COVID vaccination and encourage staff to obtain it.
Improper Food Storage and Labeling in Kitchen and Nourishment Refrigerator: Surveyors found unlabeled and undated dry storage items, an uncovered opened food box, and a scoop left inside a rice bin in the kitchen. In a unit nourishment refrigerator, staff and resident foods were mixed together without labels or dates, and the DSD stated staff used the resident refrigerator for their food while resident food from outside was also stored there.

A review of the facility's Managing COVID-19 After the End of the Public Health Emergency policy stated that the facility would provide education on the benefits of COVID vaccination to staff and encourage staff to obtain vaccinations.
Improper Food Storage and Labeling in Kitchen and Nourishment Refrigerator
Penalty
Summary
The facility failed to maintain one kitchen and one of three nourishment areas in a sanitary manner. In the kitchen dry storage area, surveyors observed an approximately 48 oz bag of Sysco Fancy Pecan Pieces inside a clear plastic bag without a label or date, an opened 5 lb box of Pearl Yellow Corn Meal in the original box without a label or date, an approximately 5 lb box of Quaker Creamy Wheat labeled only with the month and date but no year, an opened and uncovered 5 lb box of Gold Medal Chocolate Fudge Cream Icing Mix, and a food scooper left inside a brown rice bin. The dietary manager stated he would speak with the stock person about labeling and dating and stated the scooper should not have been inside the bin. At the nourishment refrigerator on the Redbud Unit, surveyors observed multiple items that could not be distinguished between resident and staff food because they were not labeled or dated. Items in the freezer included two unopened coffee ice cream containers, an unopened Trader Joe's chicken shawarma bowl, an unopened frozen rice veggie blend, and a frozen vegetables with noodles package, all without labels or dates. Items in the refrigerator included two closed restaurant-style plastic containers, a bowl of fruit in a yellow bowl covered in plastic wrap, five plastic grocery bags with items inside, and four slices of bread in a large zip-lock bag, all without labels or dates. A CNA stated the staff shared the refrigerator with residents and identified the food as staff food. The dining service manager stated staff used the resident nourishment refrigerator for their food, did not label those items, and that resident food brought from outside was also stored there. The facility's Food Storage policy required opened foods to be covered, labeled, and dated, dry bulk food scoops to be stored in a separate holder, and the Food from Outside policy assigned the dining department responsibility for monitoring refrigeration units and discarding outdated food every other day.
