During an observation of the hall 400 nurse medication cart at 8:45 AM, surveyors found 2 loose pills in the bottom of the second drawer of the cart. During a follow-up observation and interview at 9:00 AM, the LVN assigned to the cart stated the cart was her cart for the shift, that she checks it for loose pills and organizes it weekly, and that she had not done so that week. She removed both loose pills and placed them in the sharps box on the side of the medication cart for destruction. The LVN stated that loose pills in the cart could fall out while passing medications and could be picked up by a resident from the floor and possibly taken. The DON stated her expectation was that medication carts be cleaned and organized daily by the nurse or medication aide in charge of the cart, and that loose medications should not be in the carts because every medication requires a package and label. The Administrator stated medication carts should be wiped clean daily, medications organized daily, and all medications labeled, and that the staff member in charge of the cart was responsible for ensuring medications were accounted for and administered correctly. The facility policy stated medications are to be stored in the packaging, containers, or other dispensing systems in which they are received, only the issuing pharmacy may transfer medications between containers, and resident medications are to be stored separately from each other.
