Hand Hygiene Not Performed During Medication Administration: An LPN failed to perform hand hygiene before and after administering medications to two residents while touching the med cart, drawers, keys, paperwork, and resident surfaces. The residents had severe cognitive impairment and required staff assistance with ADLs; the facility policy required hand hygiene before and after resident contact and before handling medications.
A resident with a Stage 3 pressure ulcer received wound care from an LPN who failed to follow proper hand hygiene protocols. The LPN did not change gloves or wash hands after removing a soiled dressing and before applying a clean one, contrary to facility policy. Resident #11 was admitted with diagnoses including Chronic Obstructive Pulmonary Disease, Hypertension, and Adult Failure to Thrive, and had severe cognitive impairment with a self-care deficit requiring staff assistance with activities of daily living and care. Resident #83 was admitted with diagnoses including Dementia, Diabetes, and Hypertension, and also had severe cognitive impairment with a self-care deficit requiring staff assistance. During observation, an LPN prepared Resident #11's medications, touched the medication cart, multiple medication drawers, and keys in her pocket, then administered medications without performing hand hygiene. The LPN also gave the resident water while touching the straw, bedside table, and bed remote, exited the room without hand hygiene, then returned to the nurses' station, touched notebooks and the telephone, and prepared Resident #83's medications while touching the cart keys, drawers, and medication packages. The LPN administered medications to Resident #83 without performing hand hygiene before or after the medication pass. The LPN and the DON both confirmed hand hygiene should be completed before and after contact with the resident and resident surfaces during medication administration.
