During interview, the MDS Coordinator stated she was responsible for ensuring care plans were in place to address oxygen and confirmed that R25 did not have one, stating she was not sure how it was overlooked. The Administrator and DON also acknowledged that R25 should have had a care plan to address oxygen use and confirmed that no such care plan was in place at the time of review.

The facility did not provide evidence of tracking when the oxygen machine was last cleaned or when the tubing was changed. During interview, the Administrator and DON stated nursing staff was expected to clean the oxygen tank and change the oxygen tubing and water bottles weekly on Sundays, and that this should be included in resident orders, but they confirmed there were no orders for cleaning the oxygen tank or changing the tubing/humidifier bottle.
