During observation of meal plating on 12/30/25 beginning at 12:17 P.M., [NAME] #136 was seen wearing disposable gloves while using utensils to plate meals, but then used his right hand to scratch his left upper arm, scoop corn casserole, open the oven, and handle a baked potato while continuing to plate food. He also used tongs to serve mechanical soft meat, then used his right hand to shape the meat into a mound and pour barbecue sauce on it, and used both hands to sprinkle parsley onto individual meal plates. During the same meal service, [NAME] #136 was observed using his left gloved hand to push his glasses up his nose and later stood with both hands on his waist while still wearing the same gloves. He was also observed picking up oven mitts, opening the oven, removing a baked potato, and holding it with his left hand while splitting it open with a utensil in his right hand. In interview, [NAME] #136 confirmed he used the same gloves to touch multiple utensils and the oven before touching ready-to-eat food items. Dietary Manager #162 confirmed he did not change his gloves or perform hand hygiene after touching his face, his waist, oven mitts, and the oven door before touching ready-to-eat foods. The facility policy on Single-Use Gloves stated staff should change gloves whenever an activity or workstation change occurs, when contaminated after touching equipment, and after touching hair or face.
