Improper glove use during meal service

Willows At Bowling Green TheBowling Green, Ohio Survey Completed on 12-31-2025

Summary

The facility failed to ensure proper hand hygiene was practiced during meal service. During observation of meal plating on 12/30/25 beginning at 12:17 P.M., [NAME] #136 was seen wearing disposable gloves while using utensils to plate meals, but then used his right hand to scratch his left upper arm, scoop corn casserole, open the oven, and handle a baked potato while continuing to plate food. He also used tongs to serve mechanical soft meat, then used his right hand to shape the meat into a mound and pour barbecue sauce on it, and used both hands to sprinkle parsley onto individual meal plates. During the same meal service, [NAME] #136 was observed using his left gloved hand to push his glasses up his nose and later stood with both hands on his waist while still wearing the same gloves. He was also observed picking up oven mitts, opening the oven, removing a baked potato, and holding it with his left hand while splitting it open with a utensil in his right hand. In interview, [NAME] #136 confirmed he used the same gloves to touch multiple utensils and the oven before touching ready-to-eat food items. Dietary Manager #162 confirmed he did not change his gloves or perform hand hygiene after touching his face, his waist, oven mitts, and the oven door before touching ready-to-eat foods. The facility policy on Single-Use Gloves stated staff should change gloves whenever an activity or workstation change occurs, when contaminated after touching equipment, and after touching hair or face.

Improper Sealing, Dating, and Storage of Dry and Refrigerated Foods
F
F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Surveyors found that kitchen staff failed to follow facility policy and professional standards for food storage, leaving dry items such as spaghetti noodles, garlic powder, and salt unsealed, and refrigerated items such as a prepared drink, salad, and turkey lunch meat uncovered or undated. The DM and ADM both stated that all food should be sealed, labeled, and dated, that all staff are responsible for these tasks, and that staff had been trained, but observations showed food in both dry and refrigerated storage was not properly sealed or dated as required by the facility’s food receiving and storage policy.

Short Summary

Dirty can opener and contaminated dry storage bins: The DCS observed four labeled dry-goods bins with dirty rims, dry matter on the bin walls, and a scoop left inside a flour bin with flour on it. The attached can opener also had dry red matter on the blade, and the cook said it had been used that morning to open cream of corn for lunch. The DCS verified the findings and stated the can opener should be washed after each use and the dry bins and scoops should be kept clean.

The walk-in freezer was overcrowded with boxes blocking the walkway, open seafood was left unsecured, thawed chicken was placed on a plastic cover on the floor, and the cooler floor had spilled juices, debris, and a dirty cup. The cook and C-B confirmed the storage and thawing problems, and the admin acknowledged ongoing concerns with expired items and kitchen cleaning.

Unsanitary Walk-In Freezer and Ice Scoop Storage Practices
F
F0812 F812: Procure food from sources approved or considered satisfactory and store, prepare, distribute and serve food in accordance with professional standards.
Short Summary

Surveyors found that the facility did not maintain sanitary conditions in the walk-in freezer and ice machine area. Ice buildup on freezer lines was encroaching on a box of burritos, and an ice scoop holder attached to the ice machine contained standing water with two scoops resting in it and no visible drainage. The Dietary Manager acknowledged the recurring ice buildup and reported that the standing water issue had not previously been raised. These practices did not follow the facility’s policies for food safety, storage, and ice machine preventative maintenance and had the potential to affect 46 residents who consumed food from the kitchen.
