The facility failed to review the risks and benefits of bed rails with Resident #15 or the resident’s representative, obtain informed consent, and assess the resident’s ability to safely use the rails before installing bilateral enabler bars/side rails. Resident #15 had diagnoses including Alzheimer’s disease, spinal stenosis, and epilepsy, and the 01/13/2026 MDS documented severe cognitive impairment, functional limitation in range of motion to both arms and legs, dependence on staff for all ADLs, and no use of bed rails. Record review showed a right-side transfer bar was requested and installed on 10/04/2023, and the other side rail was requested and installed on 11/15/2023. There was no documented evidence that informed consent was obtained before either installation, and no documentation that an assessment was completed to determine safe use of the transfer bars. The 06/17/2024 comprehensive care plan documented total assistance with ADLs, and later care plan entries listed bilateral transfer bars for repositioning and safety, but there were no documented physician or nursing orders for the bilateral enabler bars. Observations on 01/20/2026, 01/21/2026, and 01/22/2026 showed the resident in bed or in a wheelchair with bilateral enabler bars in place. During interviews, CNAs stated the resident required total care, could not roll independently, and the bars had been placed about three years earlier by maintenance because the resident was fearful during repositioning. The UM stated the bars were discussed quarterly but the assessments were not documented, and there was no documentation for education or consent. PT stated the resident had not recently been evaluated for enabler bars and that therapy would normally complete a formal assessment for safe use. The DON stated nursing and therapy worked together to evaluate use of enabler bars, but they were not sure where this was documented.
