A resident with Alzheimer's dementia and a right-hand contracture did not consistently receive an ordered palm guard intended for contracture prevention. Staff observed the resident without the device on multiple occasions, and the guard was found on a nightstand instead of on the hand. Interviews showed the CNA, LPN, and LPN supervisor were aware of the order, but the supervisor admitted signing the TAR without always applying the device.

Resident #23 did not receive the ordered right-hand palm guard as part of treatment and care for contracture prevention. The resident had diagnoses including Alzheimer's dementia and pain in the right-hand joints, with severely impaired cognition documented on the 8/7/2025 MDS. Therapy documentation from 4/7/2025 to 5/4/2025 stated the resident was to use a palmar guard for contracture of the right-hand 3rd and 4th digits, and a physician order dated 4/15/2025 directed that the right-hand palm guard be placed by CNAs in the morning and removed at bedtime per OT recommendations. The comprehensive care plan and care instructions also directed use of the palm guard during the day, removal during meals and at night, and monitoring for placement and removal. During observations on 9/8/2025, 9/9/2025, and 9/11/2025, the resident was seen sitting in a Broda chair without the palm guard on the right hand. On 9/9/2025, the resident's right index finger had a hooked appearance, and no palm guard was observed in the room. On 9/11/2025, the palm guard was found on top of the nightstand rather than on the resident's hand. The 9/2025 Treatment Administration Record documented the palm guard as to be placed by CNAs in the morning and removed at bedtime, and the LPN supervisor signed that the palm guard was applied during the day from 9/8/2025 through 9/11/2025. Interviews confirmed the inconsistency between the ordered care and what was observed. A CNA stated the palm guard was supposed to be put on by the unit supervisor and that if it was not on by 10:30 AM, staff were to remind the nurse. An LPN stated adaptive equipment such as a brace or palm guard should have a physician order and be signed for in the TAR, and that the treatment nurse should ensure it was on. The LPN supervisor stated the resident sometimes removed the palm guard, that they should document when it was removed, and that they did not put it on on 9/9/2025 or 9/11/2025 even though they signed for it. The OT stated the palm guard was intended to prevent further decline of the contracture and should be used as ordered.
