Two residents with severe cognitive impairment and high fall risk experienced unwitnessed falls and injuries after being left unsupervised in common areas and the bathroom, despite facility policies and care plans requiring staff supervision. In both cases, CNAs assigned to monitor these residents left their posts, resulting in falls that led to abrasions and, in one case, a subdural hematoma requiring hospitalization.
