A resident with myotonic muscular dystrophy, PVD, and DM2 sustained a right calf laceration when bed hardware protruding from the frame caught the leg during an RN-assisted transfer into bed. The resident had profuse bleeding and was sent to the hospital, where 16 staples were placed. Interviews and observation showed the bolt/screw at the foot of the bed was the source of the injury, and similar bolts were still present in other rooms at the time of survey.

The facility failed to ensure the resident environment remained free of accident hazards when a bolt from the bed rail installation hardware protruded beyond the frame of a resident’s bed. Resident #1, who had diagnoses of myotonic muscular dystrophy, peripheral vascular disease, and Type 2 diabetes mellitus, was cognitively intact and able to understand and be understood by others. During assistance into bed, the resident’s right lower leg came into contact with the hardware and sustained a cut to the right posterior calf. According to the investigative record, on 05/22/2026 Registered Nurse #1 entered the room with medications and fluids and assisted Resident #1 to a sitting position at the edge of the bed. As the resident’s legs dangled over the bedside, the resident bumped the right calf on the side of the bed and bleeding began. The nurse assessed the wound, applied a dressing, and notified the on-call physician because of the extent of the bleeding. The resident was sent to the hospital for a laceration to the right posterior calf and received 16 staples. During interviews, Resident #1 stated the leg was cut on the screw at the end of the bed and that the bleeding was profuse. The resident pointed to the bolt/screw at the foot of the bed and stated it was what cut the leg. The nurse stated the resident yelled out stop and pulled back, and as the resident pulled back, the lower extremities caught on the bed where there was a bolt. Observation later showed bolts still present in multiple resident rooms, and staff interviews reflected confusion about whether the bolts had been removed at the time of the incident.
