During an observation, Resident #44 was found lying in bed with six to eight scattered facial hairs approximately three inches long on her chin and some facial hair above her upper lip that was approximately one-half to one inch long. The resident expressed that she would like the hair removed, but no one had done so. Interviews with staff confirmed that the Certified Nursing Assistants (CNAs) were responsible for addressing facial hair during resident baths, but this was not done for Resident #44. Further interviews revealed that the resident had moderate cognitive deficits and required assistance with personal hygiene. The CNAs and Licensed Practical Nurses (LPNs) acknowledged that trimming facial hair was part of their job and should have been done during bathing. The facility's policy on resident hygiene, revised in June 2022, also indicated that staff were responsible for providing nail care, shampoo, and shaving each resident on every bath/shower day. Despite this policy, the staff failed to remove the facial hair of Resident #44, who had diagnoses including osteoarthritis of the knee, muscle weakness, unsteadiness on feet, and unspecified lack of coordination.
