A resident with a history of essential hypertension, peripheral vascular disease, atrial fibrillation, and hyperlipidemia received medications intended for another resident during medication administration. The medications given in error included allopurinol, ascorbic acid, cephalexin, cetirizine, cholecalciferol, duloxetine, Kerendia, lamotrigine, senna-docusate, slow iron extended release, torsemide, and acetaminophen extra strength. The resident was alert and able to verbalize needs when assessed after the error, and complained of nausea. The medication error occurred when a CMT was preparing medications and became distracted by another resident who interrupted the pass. The CMT stopped preparing one resident’s medications, gave medication to the interrupting resident, then returned to the cart, went into the wrong room, and administered the prepared medications to the resident without following the five rights of medication administration. The CMT later realized the medications given belonged to another resident. The investigation also included a witness statement, a medication error incident report, and a root cause analysis. After the error was identified, the nurse notified the DON and the NP, and the resident was assessed with vital signs documented as blood pressure 159/75 mmHg, pulse 60 BPM, respirations 16 even and unlabored, oxygen saturation 93% on room air, and blood sugar 237. The resident complained of nausea and received anti-nausea medication per NP order. No additional reactions were noted, with no respiratory distress, changes in level of consciousness, chest pain, or alteration in mental status. The resident was transferred to the hospital for further monitoring out of precaution.
