The facility failed to ensure proper labeling and disposal of foodstuffs and failed to ensure staff wore proper hair restraints while handling food. During observation in the kitchen and dry storage areas, a staff member with a beard was seen without a beard covering, and later a dishwasher and a server were also observed with beards and no beard coverings. The facility’s Hair Restraints policy required food employees to wear hair restraints, including beard restraints, to keep hair from contacting exposed food, equipment, utensils, and linens. Observations also showed multiple food storage issues. A plastic bin containing cocoa powder was loosely covered with plastic wrap and dated only with "11/17." An opened box of macaroni and cheese had a manufacturer expiration date of 7/3/25, an opened box of penne pasta had no date showing when it was opened and had a manufacturer expiration date of 8/1/25, and an opened bag of vermicelli pasta was wrapped in plastic wrap with no opening date. In the dairy cooler, a plastic container of shredded cheese had no date indicating when it was opened or placed into the container. The facility’s policies required opened foods to be dated, stored in proper containers, and expired foods to be discarded immediately; the Registered Dietician stated the unlabeled cheese needed to be discarded because it was potentially harmful.
