The facility failed to accurately complete a Minimum Data Set (MDS) assessment for one resident who had dementia, a BIMS score of 2/15 indicating severe cognitive impairment, and was dependent on staff for all activities of daily living. Review of the resident’s face sheet and MDS showed the resident had been admitted to the facility and had previously been on hospice services. During observation, the resident was sitting in a chair in her room with family at bedside, and the family reported that hospice had been discontinued around September 2025 because the resident’s health had improved. Record review and staff interview showed the resident had an annual MDS completed after she graduated from hospice services, but the assessment incorrectly marked that she was receiving hospice services. MDS staff verified that the resident had been discharged from hospice on 11/5/25 and stated the MDS should have been marked no for hospice services. The MDS staff also stated the assessment was incorrect and did not know why it had been entered in error. The DON later verified that the resident had discharged from hospice services and that the MDS should have reflected that status.
