One resident had an order for continuous O2 at 1 L via NC and diagnoses including vascular dementia and heart disease; the other had an order for O2 at 2 L via NC PRN for SOB and diagnoses including HF, hypotension, AFib, PE, and SOB. Observations showed both residents receiving O2 without the required signage outside their room doors, and the DON confirmed the missing signs.

The facility’s Oxygen Administration Policy stated that while oxygen is in use, No Smoking signs will be posted at the entrance to the room and that the warning sign should be placed on the resident’s room door or in another appropriate location. Resident #99 was admitted with diagnoses including orthostatic hypotension, vascular dementia, and heart disease, and had a physician order for oxygen at 1 liter per nasal cannula continuously. Observations on 06/22/2026 and 06/23/2026 showed the resident in the room receiving oxygen, but no signage was posted outside the room door. Resident #125 was admitted with diagnoses including heart failure, hypotension, atrial fibrillation, pulmonary embolism, and shortness of breath, and had an order for oxygen at 2 liters per nasal cannula as needed for shortness of breath. Observations on 06/22/2026 and 06/23/2026 showed the resident in bed receiving oxygen, but no signage was posted outside the room door. During observation with the S2DON, the absence of signage was confirmed for both residents.
