Food Service Safety Deficiency

Forest Haven Nursing & Rehab Ctr, LlcJonesboro, Louisiana Survey Completed on 06-24-2026

Summary

The facility failed to ensure the kitchen prepared and served food in accordance with professional standards for food service safety, affecting 121 diets served from the kitchen. During lunch meal observation, residents were served herb baked chicken, riced dressing, squash and zucchini, and chocolate and strawberry cake, with alternate foods available including pulled pork, mashed potatoes, and cabbage. For the mechanical chopped diet, residents were served chopped chicken. When trays were being served from the steam table, no temperatures had been taken on the rice dressing, mashed potatoes, pulled pork, cabbage, chopped chicken, pureed chicken, pureed rice dressing, or pureed squash and zucchini before the meal was served. Further observation showed S7 Dietary used bare hands to place chocolate and strawberry cake into a blender to make a pureed dessert. During lunch service, S6 Cook used bare hands to handle bread from a package and cheese from a plastic bag to make grilled cheese sandwiches. The same staff member also used bare hands to prepare a chef salad by handling lettuce, sliced turkey, tomato, and grated cheese directly with her hands. In interview, S5 Production Manager confirmed the food temperatures were not taken for the listed steam table items before serving and agreed that S6 Cook should not have used bare hands to make the grilled cheese sandwich or the chef salad.
