However, during the surveyor’s review of staff certification, policy, and staff interviews, it was determined that this certificate did not reflect a legitimately completed training by Staff A. In interviews, Staff A reported he had trouble passing the DAA test, took it five times, and never achieved a passing score. He stated that Staff C, the administrative assistant, told him she would “let it go,” and he believed it was possible she had his login information and did not know what happened with his test after she “okayed” it. Staff B, another maintenance staff member, confirmed that Staff A struggled with the DAA course and that Staff C said she would “take it from there,” though he did not know what that meant. Staff C initially denied taking a test for another employee but, after further questioning, admitted she took the DAA test for Staff A because he was struggling and had taken it many times, and acknowledged that no one else at the facility was aware she had done so. The Executive Administrator stated that employees should take tests themselves and that he became aware of the situation with Staff A and Staff C on the day of the interview.
