Food was not stored and prepared under sanitary conditions in the kitchen, based on observations, staff interviews, record review, and review of the facility policy titled Date Marking for Food Safety. During a kitchen tour with the Certified Dietary Manager, the trash can lid next to the handwashing sink had a black substance, the paper towel dispenser had a slick substance, and the handwashing sink bowl contained a gray scum-like substance. A can opener also had a black substance present, and one trash can near the fryer was uncovered. In the dry pantry, multiple food items were observed past their expiration dates, including individual packages of thousand island salad dressing, golden Italian dressing, tartar sauce, and several seasonings such as rubbed sage, ginger, Italian seasoning, ground mustard, crushed red pepper, ground thyme, chopped chives, freeze-dried chives, whole tarragon leaves, and ground nutmeg. An open bag of potato logs in the ice cream freezer was unlabeled. The CDM and ADM confirmed the unclean observations and expired items, and the CDM stated dietary aides failed to check the seasonings and that no log was maintained to track seasoning expiration dates.

The facility failed to assure dietary department equipment was in safe working condition when the kitchen steam table was found to be in disrepair and not maintaining food at required hot-holding temperatures. During observation, food on the steam table was measured below the facility’s stated standard of 135 degrees Fahrenheit or greater, including gravy at 132 degrees Fahrenheit, hot dogs at 130 degrees Fahrenheit, chicken tenders at 120 degrees Fahrenheit, and chocolate bread pudding at 120 degrees Fahrenheit. The Dietary Manager and Registered Dietary Manager confirmed the temperatures were not within state regulatory requirements and stated staff may not have set the steam table at the highest level. Later observations showed the same problem persisted, with grilled cheese at 95 degrees Fahrenheit, mashed potatoes at 120 degrees Fahrenheit, puree beef at 113 degrees Fahrenheit, and chopped beef at 100 degrees Fahrenheit. The Administrator stated he was not aware the steam table was not functioning properly until the issue was identified, and the Dietary Manager stated she was unaware the steam table was not working properly until the second set of temperature checks. The Administrator later reported the steam table had not been holding temperature on the serving side for well over a month.
