Resident #6 was admitted on 9/11/2024 and had diagnoses of type 2 diabetes mellitus, major depressive disorder, insomnia, and anxiety disorder. During observation on 6/3/2026, he was sitting in a reclining chair in the corner of his room with the television off and said he was feeling okay, had good days and bad days, was independent with ADLs with minimal staff assistance, and could not name all of his medications or what they were treating. The resident’s active physician orders included Wellbutrin XL 300 mg daily for depression, Citalopram 20 mg daily for depression, and Trazodone 50 mg at bedtime for depression. The most recent annual MDS assessment, with an ARD of 5/5/2026, showed a BIMS score of 08 out of 15, indicating moderate cognitive impairment, and included anxiety and depression diagnoses. Antidepressant medication was administered during the look-back period, the Psychotropic Drug Use Care Area was triggered, and the Psychotropic Drug Use Care Planning Decision was coded as yes. Review of the active comprehensive care plan showed no care plans addressing the resident’s depression, anxiety, or insomnia, and no care plan for antidepressant use or monitoring for adverse side effects related to daily medication use. A behavioral health progress note dated 5/20/2026 stated the resident was depressed and feeling down, and that the medication regimen of Citalopram, Wellbutrin, and Trazodone was being used as an augmentation strategy for depression. On 6/3/2026, the resident was again observed seated in the same location, with the television off, looking out the window, and he stated he did not like the facility’s many activities and enjoyed being alone. The SSD confirmed there was no active care plan for depression and stated she added one on 6/5/2026.
