During tray line observation, three mixed fruit cocktail desserts were sampled and each was recorded at 46 degrees Fahrenheit. The Dietary Manager placed the dessert tray in the walk-in refrigerator and stated the tray would not be used until the temperature was rechecked and lower than 40 degrees Fahrenheit, and that in the future fruit cocktail cans would be placed in the refrigerator a day before serving to ensure the temperature was below 40 degrees Fahrenheit at serving time. In a later kitchen observation, two maintenance/construction staff were seen in the kitchen without hairnets while speaking with the Dietary Manager, and the Dietary Manager acknowledged the concern and stated hair nets were available at every kitchen entrance door.
Improper Catheter Bag Positioning During Hygiene Care

The facility's policy also required catheter care to reduce bladder and kidney infections. The DON confirmed during interview that the bag should remain below the bladder to prevent urine reflux, especially given the resident's current UTI diagnosis.
