Improper Catheter Bag Positioning During Hygiene Care

Staff failed to provide appropriate catheter care for a resident with an indwelling urinary catheter. During hygiene care, a CNA placed the resident's urinary drainage collection bag and tubing on the bed between the resident's feet, positioning it above the level of the bladder. The surveyor observed backflowing urine in the tubing and questioned the CNA, who stated it was acceptable because the bag had been emptied. The CNA continued hygiene care with the bag and tubing remaining above the bladder level. The resident involved had a diagnosis of bacteriuria and a current order for antibiotics to treat a urinary tract infection. Physician orders specified that the urine collection bag should be kept covered and attached below the level of the bladder at all times. The facility's policy also required catheter care to reduce bladder and kidney infections. The DON confirmed during interview that the bag should remain below the bladder to prevent urine reflux, especially given the resident's current UTI diagnosis.
