The DON and the MDS Coordinator confirmed that the residents were admitted during the annual influenza season and should have received the CDC's vaccine information along with the consent or declination forms. The lack of documentation indicates a systemic failure in the facility's process for ensuring that residents are educated about and offered the influenza vaccine, as required by their policy. This oversight puts residents at risk of developing influenza due to the lack of proper immunization education and administration.
