The facility failed to ensure Physician’s Orders were transcribed without error and consistently followed for two residents. One resident was admitted with type 2 diabetes mellitus and an acquired absence of the left great toe, had a BIMS score of 15, and was receiving daily insulin injections. The resident’s orders included a sliding-scale fast acting insulin regimen and an order to notify the MD for blood sugar results less than 60 or greater than 400. On 12/11/2025, the resident’s blood sugar was documented as 469, which was above the ordered parameter. Although the elevated blood sugar was reported to the charge nurse, the record did not show documentation that the provider was notified as ordered. The DON stated the provider had been contacted, but the PA reviewed messages and the electronic record and reported there was no evidence of being contacted for the out-of-parameter blood sugar result. The MD also did not recall being contacted. The resident’s Progress Notes did not contain documentation of provider notification for the 469 blood sugar reading. A second resident was admitted with ulcers on both heels and the left toes, along with diagnoses including atherosclerosis of native arteries of the extremities with intermittent claudication, type 2 diabetes mellitus, traumatic subcutaneous emphysema, peripheral vascular disease, and chronic kidney disease. Transfer orders indicated two antibiotics per day for seven days, but the medication was entered on the MAR as one tablet twice a day every seven days. The resident received the initial dose on admission and then did not receive another dose until the DON identified the transcription error and corrected the order, resulting in two missed days of antibiotics. The MD confirmed the nursing staff made the transcription error when entering the order.
