Summary

The facility failed to provide Resident #9 with an ongoing program of activities based on her comprehensive assessment, care plan, and preferences. Resident #9 was an [AGE]-year-old female with diagnoses including unspecified dementia with mood disturbance, cognitive communication deficit, and anxiety disorder. Her Significant Change MDS assessment showed a BIMS score of 9, indicating moderately impaired cognition, and her activity assessment reflected that she preferred not to participate in group activities but did enjoy reading, watching television, the news, and coloring. The activity assessment also indicated she was to receive one-on-one in-room activities three times per week. Record review and interviews showed Resident #9 did not receive the planned in-room activities during April 2026, May 2026, or from June 1 through June 18, 2026. Her care plan identified her as dependent on staff for activities, cognitive stimulation, and social interaction related to physical limitations, with interventions to introduce her to residents with similar interests and invite her to scheduled activities. During observation on 06/16/2026, she was lying in bed, did not respond to much conversation about activities, stated no when asked if she attended group activities or if anyone came to her room to visit or do activities with her, and said yes when asked if she would enjoy someone coming to her room to do an activity. There was no sensory stimulation in her room. Interviews confirmed the lack of activity provision. A CNA stated she did not recall observing anyone in Resident #9's room assisting with activities and noted that residents without activities could become bored or lonely. The Activity Director stated Resident #9 was on the in-room activity program but did not receive in-room activities during the months reviewed because she forgot to add her name to the in-room activity list. The Administrator stated it was very important for residents unable to come out of their rooms or participate in group activities to receive activities in their rooms and expected all activities to be documented daily, with in-room activities started immediately when assessed and based on resident preferences.
