Summary

The facility failed to provide an ongoing activities program based on the comprehensive assessment, care plan, and resident preferences for 1 of 17 residents reviewed for activities. Resident #11, a [AGE]-year-old male admitted on [DATE], had diagnoses including anxiety disorder, chronic pain syndrome, scoliosis, and end stage heart failure. His admission MDS dated 06/08/2026 showed a BIMS score of 13, indicating he was cognitively intact, and identified that doing his favorite activity, listening to music, and keeping up with the news were somewhat important to him. His comprehensive care plan dated 07/30/2025 directed the facility to provide a program of activities of interest and to assist him to attend activity functions. During interview, Resident #11 stated he stayed in bed and did not have in-room activities. He said someone had spoken with him about his activity preferences when he first arrived, and he would like a word search book, but he had not told anyone. The Administrator stated he had just hired a part-time person and was looking for the in-room activity logbook from the previous activity director. He later stated he expected the ADs to provide in-room activities and could not find the log of in-room activities. The CHR E stated the AD position was open, the facility was seeking a qualified candidate, and the previous AD had been employed from 3/31/2026 to 5/12/2026.
