The facility failed to ensure that Resident #21 was offered an ongoing program of activities. The resident was admitted on 01/05/2026 and had diagnoses including depression and muscle weakness. A quarterly MDS with an ARD of 04/20/2026 showed a BIMS score of 15, indicating intact cognition. The resident’s care plan identified a need for staff support with emotional, intellectual, physical, and social needs, and interventions directed staff to provide a program of activities that matched the resident’s interests and encouraged choice, self-expression, and responsibility. The DON stated she expected the Activities Director to ensure all residents received some type of activity on an ongoing basis, and the Administrator stated she expected the Activities Director to document activity participation for all residents.
