Water Heaters Not Maintained in Safe Operating Condition

St. Teresa Nursing & Rehab CenterEl Paso, Texas Survey Completed on 07-16-2026

Summary

The facility failed to maintain essential equipment in safe operating condition for 4 of 12 water heaters. Review of the water temperature logs showed hot water temperatures had not been checked weekly since May 2026, and the Maintenance Director stated he had been having problems with hot water in the community shower rooms because one tankless water heater was down in 4 of 4 resident halls. He also stated he was still trying to obtain 3 quotes required by company policy and had only obtained two quotes, one of which was blank and did not include a date or vendor name. During observations and interviews, the Maintenance Director confirmed that one tankless water heater in the 200 Hall, one in the 300 Hall, and one in the 500 Hall had not been working for several weeks and were turned off, with each tank labeled "Replace." Hot water temperatures measured in shower rooms were 88.3 degrees Fahrenheit in the 200 Hall, 103.3 degrees Fahrenheit in the 300 Hall, and 84.7 degrees Fahrenheit in the 500 Hall. The Maintenance Director stated he did not have written documentation showing water temperatures in the resident halls, kitchen, and laundry had been checked since May 2026, and the Administrator and Regional Compliance Nurse confirmed the checks had not been done since that time.
