Summary

The facility failed to ensure incontinent care was provided in accordance with appropriate treatment and service practices to prevent urinary tract infections and to restore continence to the extent possible for a resident who was always incontinent of bowel and bladder. The resident had a diagnosis of hemiplegia and a BIMS score of 03, indicating severe cognitive impairment. The care plan identified a problem of incontinent bladder related to functional incontinence and directed staff to provide proper perineal care and use barrier creams to protect the skin from urine and feces exposure. During observation, two CNAs provided incontinent care for the resident, and one CNA used a back-and-forth motion to wipe the resident instead of a front-to-back motion. The CNA also wiped the resident's penis from base to tip instead of tip to base. During interview, the CNA stated the proper motion was front to back or tip to base and said she did not realize she was using the wrong motion. The DON stated the correct motion was front to back to prevent contamination of the urinary tract with fecal matter and prevent infection. The facility policy for perineal care directed staff to wash the perineal area starting with the urethra and working outward.
