Failure to Transmit Completed Discharge MDS Assessments
Summary
The facility failed to ensure that MDS assessments were completed and submitted timely to the CMS system for 1 of 2 sampled residents, involving two discharge-return not anticipated MDS assessments for Resident 36 and Resident 40. Resident 36 was admitted with diagnoses including asthma and lymphedema, and Resident 40 was admitted with diagnoses including peripheral vascular disease and type 2 diabetes mellitus. The record review showed Resident 36's discharge MDS had an ARD of 3/21/2024 and was completed on 3/07/2026, but it was not transmitted to CMS when completed. Resident 40's discharge MDS had an ARD of 3/17/2026 and was completed on 3/03/2026, but it was not transmitted to CMS when completed. During interview and concurrent record review, the MDS Nurse stated she was responsible for ensuring all residents' MDS assessments were completed and said the facility had 14 days from discharge to complete and submit the discharge MDS to CMS. She stated the discharge MDSs for Resident 36 and Resident 40 had been completed on time, but the DON had signed and selected the option not to transmit them to CMS. During interview, the DON stated the discharge MDSs should have been transmitted immediately after completion and said she did not notice that she had chosen the option not to transmit them to CMS when she reviewed them as complete. The facility policy titled Electronic Transmission of the MDS stated that all MDS assessments and discharge and re-entry records are completed and electronically encoded into the facility's MDS information system and transmitted to CMS IQIES in accordance with current OBRA regulations.
