Kitchen Food Safety and Sanitation Deficiencies
Summary
The facility failed to ensure food safety and sanitation requirements were met in the kitchen during observations, interviews, and policy review. The facility served food prepared in the kitchen to 168 of 172 residents, and surveyors observed multiple food storage and sanitation issues during the kitchen tour with the DSS. These included opened bags of chicken breasts in Freezer A that were not labeled with opened or use-by dates, juice boxes connected to the dispenser without opened dates, and a box of prune juice labeled with the date 3/10/26, with the DSS stating she did not know whether that date was the received or opened date. Surveyors also observed multiple refrigerated food items past their use-by dates, including Italian dressing labeled 6/27/26, jelly labeled 6/27/26, ham slices labeled 6/28/26, and yogurt and pudding labeled 6/28/26. The DSS verified these findings. In addition, multiple kitchen items and food-contact surfaces were observed with debris, food residue, stains, or other contamination, including plastic bin contents, a mixer bowl, a meat slicer, a utensil drawer, a pizza slicer, a stained cutting board, and a blender with white food residue during puree preparation. The DSS verified the equipment and bins should be clean, and Dietary Aide 1 was observed removing the blender to be washed. Surveyors further observed chipped spatulas, a melted green plastic container lid, a cracked blue plastic container lid, and a heavily marred cutting board. Several food-contact items were also observed wet after cleaning, including water pitchers, cups, plastic bins, trays of bowls, and scoopers stored while still visibly wet. The emergency food supply room did not have a thermometer, and the DSS stated the room temperature was not being monitored after the emergency food supply was moved there a week earlier. During puree preparation, Dietary Aide 1 was observed removing gloves and donning new gloves without washing hands first, including while preparing tamale pie puree and pureed carrots. The refrigerator fans in Refrigerator A and Refrigerator B were also observed with blackish and fuzzy residue, and the Administrator, RD, and DSS were informed and acknowledged the findings.
