Dialysis care records, fluid monitoring, and bedside emergency kit deficiencies
Summary
The facility failed to provide safe, appropriate dialysis care and services for residents receiving hemodialysis. The report identified deficiencies involving incomplete dialysis communication records, lack of documented monitoring of dialysis access, inaccurate monitoring of fluid restriction, and failure to keep an emergency dialysis kit at a resident’s bedside. The facility’s dialysis management policy stated that residents requiring dialysis care should receive services consistent with professional standards of practice and a comprehensive person-centered care plan meeting the resident’s goals and preferences. For one resident with ESRD who was dependent on renal dialysis, the record showed a left upper arm AV shunt/AV fistula and a schedule for hemodialysis three times weekly. The dialysis communication record for multiple treatment dates was incomplete because the access site location was left blank. The medical record also did not show licensed nurse documentation of bruit and thrill checks on days the resident did not receive dialysis treatments, and there was no physician’s order found for that monitoring. The resident’s intake and output orders were also not accurately reflected in the MAR, with inconsistent entries for shift intake and 24-hour intake documentation. For two other residents with kidney disease and dialysis-related diagnoses, physician’s orders required a 1500 ml fluid restriction with nursing and dietary allocations and monitoring of intake and output every shift. Their MARs showed only check marks for fluid restriction monitoring on each shift, without documentation of the specific amount of fluid consumed. One resident was observed with a 500 ml water pitcher at the bedside, and staff confirmed that only tray fluids were being recorded on the intake log, not the bedside water pitcher. In addition, for another resident receiving hemodialysis, the emergency dialysis kit was not observed at the bedside, and an LVN confirmed that no kit was present.
